Here at Mosaic Periodontics, we will verify the benefits on the plan, submit claims to your insurance on your behalf and help to navigate the benefits you have with your unique policy. We will provide you with the necessary information to guide you in the process. We highly recommend that you are active in understanding your benefits as well. As much as we try to give the most accurate information, please understand that insurance polices are unique to the Employer with possible exclusions we may be unaware of and can’t change.
If you would like us to file your claim, please complete all information on the patient registration form so that the claim can be processed. Below are some of the many companies we work with on your behalf;

Please keep us aware of any changes to your insurance plan so that we can keep our estimates of coverage as accurate as possible.
Keep in mind that even if a claim is in process and hasn’t been completed, your portion (co-pay, etc) of the costs are due at the time of service.
If the insurance company rejects a claim, we will assist in negotiating the claim for up to 60 days. However any unpaid balance is ultimately your responsibility regardless of the claim resolution.
